The Tangipahoa Parish Sheriff’s Office is asking for the public’s help locating a missing woman who may be in distress after leaving a recovery facility in Hammond.

Authorities say 30 year old Nicole Middleton of Slidell walked away from the LynHaven Retreat property on Old Covington Highway on May 12. Investigators believe she may not have any personal belongings with her, including identification or a cellphone.

Middleton is described as having brown hair and brown eyes. She was last seen wearing a gray shirt, blue jeans, and sandals.

Officials also stated she receives treatment for a mental illness, raising concerns for her safety and wellbeing.

Anyone with information on her whereabouts is urged to contact the Tangipahoa Parish Sheriff’s Office immediately.
